No, not necessarily. You can make an appointment yourself without being referred by another physician.

We are located in Warren, Michigan on Ryan Road between 8 and 9 Mile. Click here for directions to our office.
1What should I bring to the office?

What you need to bring to the office depends upon the type of appointment you have, but here are a few items to consider:

  • Insurance information (especially if your insurance has changed since your last visit)
  • Referrals from other physicians
  • Lab results
  • List of medications (names, dosage, etc.)
  • Any online forms that you have already completed
  • A guardian if you are a minor
  • Change of address information.
1How early should I arrive?
Please try to arrive approximately 15 minutes before your appointment. This should give you enough time to complete any necessary forms if needed.
1What should I eat before my appointment?
